Pickaxe disappeared while shift mining

I was mining iron ore in the most recent version of Minecraft. My pickaxe was at near full durability, diamond with Effeciency V, Fortune 3 and Unbreaking 3. When I broke the Iron Ore the pickaxe just disappeared. There wasn't a breaking sound or anything, it just disappeared. Reloading the game, and quitting the game and relaunching it had no effect.

 

Based on additional testing I have found that it happens regardless of what enchantments are on the pickaxe
