Game crashes randomly with error: "Missing palette entry for index __"

Players on my server are randomly having their game crash client-side. Unfortunately we were not able to detect a pattern of behavior that triggers the crash. However the log is always the same - the error message states "Missing palette entry for index __", where the blank is some number. The attached logs are from myself and one other player on my server.
