Voluntary Exile advancement received but Bad Omen not generated.

I shot a Pillager Captain with an arrow, he fell from a high place and died.  I received the Voluntary Exile advancement but was not given bad omen.  I think because he died from the fall I should not have been given the advancement.  Either way, something is bugged.
