Glitching Blocks

On an online server I am in, there is a glitch where some blocks will get replaced when trying to break them (ex breaking cobblestone with a pickaxe replaces the cobblestone instead, or trying to pick sweetberries from a sweetberry bush replaces the berries without dropping any). In addition one of the doors will not open right. As seen in the attached file, the bottom half of the door is closed while the top half is open. The top half of the door may also be closed while the bottom half of the door is open.

Is this a common mishap that happens to anyone using 1.16.3? (The Affects Versions would not allow me to put down 1.16.3.)
