MC-200280Throwing an Ender Pearl up while a block is on top while on a ladder causes you to go through the block
This is how most people get to the Nether roof in survival they place down ladder, climb up to the bedrock and then throw an ender pearl upwords. Suddenly you go through the bedrock.
