If killed while in a nether portal player will still be moved to the overworld and their hand will be missed place.

This can be replicated by placing a weighted pressure plate with a redstone torch attached on the block blow it. The redstone torch should power a command block with the command "kill @p". Then drop an item on the weighted pressure plate set gamemode to survival and walk into the portal.
