not respawnable death

i went in creative and gave myself a 255 health boost for forever and was playing with it for a while then later i left minecraft in creative - when i reopening the game i went in survival and was later killed randomly and can't re-spawn - i reopened minecraft and was still dead without the ability to re-spawn - i was NOT in game-mode 2
