"Ticking memory connection"

After installing and converting Skyblocks and playing for a while, the game froze suddenly, then crashed completely. I have got optifine, inventory tweaks and minecraft forge installed and so they may be the cause, or it could be the artificial environment of skyblocks. I'm not sure.
