Player falling into the void from the edge of loaded chunks, mobs frozen

When I activate my elytra I am stuck in 'flight mode' and cannot jump 1 block. 

When I break a block its item does not drop. 

Flying over an unloaded chunk using an elytra also results in ending up in the void. 

I didn't take any damage from any mobs or lava but did when I fell into the void. I saved before dying so I don't know what would have happened after dying.
