Name tagged hostile mobs despawning; Name Tag duplicating

Even if renamed with a Name Tag, skeletons – protected from sunlight – keep despawning when I walk away.
Difficulty set to Hard.

Moreover, once the Name Tag is used on the mob, sometimes a copy of it – with the same name – will appear in my inventory. The mob still keeps its new name.
