Block structure changed during mining

UPDATE on Icons

I was playing a new game with the same parameters when I had a new problem. The items in my inventory were corrupted and changed including the display of what I was holding. When I hit the E key for my inventory, the display still shows the corrupted view. When you place a block, it looks like all the other blocks, the problem seems to be isolated to the view of the inventory. See attached screenshots.

When striking blocks with corrupted tool, the block goes from a normal looking block to a corrupted block. See Screenshots
