Tools disappearing from hotbar when used once.

It was the middle of the night. I was hunting mobs and my sword just disappeared. It had almost full durability and was nowhere near broken. I switched to my axe to get rid of the mobs and that broke as well! I disconnected from the server and came back and my tools were still gone and they are still gone to this day. All I have remaining now is my pickaxe. This also happened to my friend on the same server. His sword broke mid battle and recently he used a bucket and the bucket disappeared from his inventory, It did not fly out because of a full inventory or anything, it just disappeared.
