Skeleton's shoots me with arrow, Item in my hand disappears

At random a skeleton will shoot an arrow and me, and items like my swords or buckets will disappear. They aren't dropped or in my inventory, they are just gone. I've lost 2 swords and a bucket with in 5-10 mins of game play. The swords were fairly new.
