Disappearing weapons while being hit by mob

This happened to me twice so far. Once in the Nether and the other in the End. I was attacking a skeleton in the Nether with an enchanted ax, and when it shot me, my ax disappeared. While attacking an enderman with an enchanted sword, my sword disappeared. It was not on the ground or back in my inventory.
