OpenGl error

at random times while playing the game will freeze up and the screen will go black. i have tried everything from updating my graphics drivers to only using one monitor, there are no mods, resource packs or data packs being used. i have included the crash report as well as the output log. i have been having this issue since 1.11 on a different computer
