Items in the main inventory bar disappear and is REPLACED by a STACK of either iron or gold ore

This only happend when im in creative mode i dont know how it replaced by gold and iron ores but i never really saw how its replaced for example;

I have a diamond sword and many other items in my main inventory bar and like seconds or minutes later its REPLACED by iron or gold ores
