Hard Mode Bug

After I eat one meat and fill hunger instantly i have to eat another one because of my hunger depleting. Well there is no hostile mobs even though I have it set to hard mode. went to nether and hear ghast but nothing was there so then I quit and reopened and it was there but disappeared. Items like from trees that fall like saplings sticks disappear Instantly.
