Raid bar randomly disappears and is inaccurate

The raid bar will randomly disappear during a raid and sometimes disappears and never comes back, even when I'm in the village radius. The raid bar will also sometimes display as "Mobs remaining: 1" (or 2) even though there's more than that alive in the raid.
