Mobs mysteriously die and cant place blocks

For some reason, there are spots where it thinks that there is a mob there but there isn't if a mob or player goes into that area they die(player has been squished too much) and it is impossible to place blocks in that place as they are taken from the inventory but are not placed

 

How it happened?

 Probably from killing a lot of mobs on the same block
