Commands disappear from command blocks randomly

I'm not sure what is causing this. for whatever reason, specific command blocks seem to just erase their own commands randomly. They are all either chain-always active or repeat-needs redstone. It's not an issue of me pressing cancel instead of confirm accidentally (I've tested), it just seems to be a random ocurrence.
