Furnace output disappears using hopper mine-carts

Sometimes one or two items from the output from a furnace will disappear whilst using hopper mine-carts. Used hopper mine-carts and hoppers for both inputs. There is a 50 to 75 percent chance that 1 or 2 items will go missing. I have checked every possible and impossible place the item can be and has not been found.
