Visible Monster & Items

every time i die and re spawn, The mobs and items become visible. I can still pick up the items, but i cant hurt the mobs, but they can still hurt me.  Also the only way i can fix the issue is by going back to the main menu and go back to the saving. It will still come back when dyeing and re spawning.
