This bug causes the game to turn into extremely basic textures (either white squares/blocks or green ones)

This bug happens randomly in all gamemodes, difficulties online, offline, single player and multiplayer. I have got filmed proof of this bug at this link http://www.youtube.com/watch?v=OFx_5YytRB4
please sort it out, it occurs very often disrupting gameplay.
