CustomName tag doesn't work

summoning an entity which uses the "CustomName:"[name_here]"" tag, the command will result with the command failing.
i've checked the letter capitalization, all the command pages to make sure that the tag hasn't changed, whenever i type the command correctly, it displays "Unable to summon entity"
