Piston not Working after mono-pulse

When attached to a side-by side mono-stable system, after the first pulse, the piston becomes dysfunctional. Destroyed redstone and placed again, still dysfunctional. Removed redstone around it, simply placed a redstone torch next to it, still doesn't work. I have a video if need be
