Cannot destroy blocks or use tools

When I left-click, my character will not destroy blocks or use his tools. The mouse is not broken, it is within the game.
To re-create
1. Enter Nether portal
2. Before you are sent to the loading screen, hit a block through the portal
3. Enter the Nether unable to left-click
4. Whatever you hit the block with might disappear
5. It does this for every world that you have
