Minecart Trains randomly derail

The last minecart in a "train" jumps off of the rails when being pulled by a minecart with furnace and running over a switch. Also, the coupling process is dodgy and sometimes will not work.

A side note: Having an item to couple up Minecarts would be really great and a feature I liked to see since beta.
