Items are thrown when moving from inventory to toolbar

While attempting to move an item from inventory to toolbar the item is thrown. I can click and pick item up from inventory but when I click to place the item into toolbar the item is thrown and I have to pick it up to get it into the toolbar. Touchscreen mode is turned off.
