Glitching worlds

When ever I enter a world (Single player or multiplayer)after about 5 minutes of playing my world graphics glitch [as seen in attachment ]

What I expected to happen was...:
Normal game function.

What actually happened was...:
My graphics glitched

Steps to Reproduce:
1. I go on Minecraft
2. I go on a world
3. Play for 5 minutes
4. My graphics glitch
