Trident disappears or glitches when ever it has the loyalty enchant and player dies.

When I throw the trident (the trident has loyalty III) but befor the trident comes back, I drown, the trident either glitches like it's trying to come back and repeatedly comes back but never fully does. Or, the trident simply disappears.
