Blacked out head when using skin in-game

I've used Skindex to create a custom skin. When I launch Minecraft, my skin appears perfectly under the "Skins" tab, but upon entering gameplay, the head is just one jet-black block. None of the other body parts are affected. I've tried re-uploading the skin to no avail.
