Game crashes after connection with Multiplayer and Singleplayer worlds

After successfully logging, I select a multiplayer server or a single player world, world loads in, however as soon as I begin interaction (walking, typing, etc) game crashes giving error "An unexpected issue occurred and the game has crashed. We're sorry for the inconvenience." Attached is a picture of our error and the debug window. This occurs on all minecraft updates we've tried (1.8 to 1.12)
