Creative decorations inventory banners imitate banners from saved tool bars.

Banners in the creative menu "Decorations" had changed their appearance to match ones that were being stored in the saved tool bars tab. They would stay that way even when changing worlds, removing banners from saved tool bars, reloading the resource pack and reverting to default resource pack. The issue was resolved by restarting the game and could not be recreated.
