Player TPs back after moving

Sometimes, when I try to move, I just get teleported back to where i was before i moved. Also, this reverts blocks you place/mine. There is not really a pattern to this, it randomly happens, forcing you to reconnect to be able to move. This seems to happen more often in the nether.
