Unloaded blocks and chunks

On MC servers, blocks do not load in, and when in contact with those blocks (smashing my head on the wall), they glitch out my character. I have no idea what is wrong, but it usually happens on Hypixel, and rarely my Singleplayer worlds, but does happen.
