Random and erratic mouse movements in game.

When I get into game everything works fine and my mouse moves fine and normal. When I hold down either mouse button and move my mouse or click either mouse button as i'm moving I get these weird and random erratic mouse movements which make the game unplayable. This happens in Vanilla minecraft and in modded minecraft with Optifine and Shaders.

I've reinstalled the game, java and rebooted tons of times. I've reinstalled graphics drivers, played with graphics settings on NVIDIA control panel and still nothing is working. Attached is a crash report from holding F3 and C for 10 seconds in Vanilla Minecraft with the error present.
