/whitelist remove not working if name was changed since addition

1) Add player to the whitelist.
2) Have this player change their name
3) Try to do /whitelist remove <NewPlayerName>

It will fail since the whitelist file still contains the old name, but Minecraft searches for <NewPlayerName> in the whitelist and not for the player's UUID.
