Badly done skin head

Attempting to use a newly made skin (template shown below) (Steve skin format), I had ended up with a mistextured head and the picture shown in the head location was nowhere else on the skin. Both a picture of my skin (the entire head is badly done, only the front is shown) and the template are shown below.
