Hostile Mobs drop custom items not on loot table

Does this issue only occur on Bedrock Dedicated Server or it can be reproduced on Realms/Local Worlds? What platform/Device are you using?

This is on a Bedrock Dedicated Server. I am on a PC, but other users are on Xbox, Playstation, Mobile and Switch.

Is there a way I can upload it so it can't be downloaded by others? This is a custom add-on I made myself for my server and I don't want other users getting the code.

I've changed security level to private, now your ticket and attachements will be visible for you and moderators/helpers only.

Thank you Kinga, I have attached the resource and behaviour packs. So far the swords are the only items that seem to be dropping upon mob death.

We've also noticed recently that when players die, they also drop swords occasionally. Even if they did not have it in their inventory.
